I need your help identifying more examples of mistake-proofing in health care.  If you know of any, please share them.

How to share your example:

Method 1: Create a new example in Volume 2. Volume 2 is a wiki of examples of mistake-proofing in health care. According to Wikipedia, "a wiki <WEE-kee> is a website that allows the visitors themselves to easily add, remove, and otherwise edit and change available content, and typically without the need for registration. This ease of interaction and operation makes a wiki an effective tool for mass collaborative authoring." Method 2: Submit an example using the on-line example submission form

Method 3:  Email it. You can email descriptions and attach JPG or other common graphics files.

Method 4: Fax it. You can fax descriptions and drawings to John Grout at (706) 238-7854

Method 5: Mail it. You can mail a descriptions and photos to John Grout, Campbell School of Business, Berry College, Mt. Berry GA 30149-5024
